This section covers applicable health insurance procedures to be completed when an infant is born.
Please follow these procedures carefully, since some have submission deadlines while others require certificates or additional documents.
All documents are in PDF format. Adobe's Acrobat Reader software is required to view or print them. If Acrobat Reader is not already installed on your computer, please download and install the latest version now.

* Please attach a "written explanation" if 3 or more months have elapsed since birth.
Although no special format is required for this written explanation, you must include at least the following items:

(1) Addressee: Chairman of the Sony Health Insurance Society
(2) Subject insured individual: code and number of health insurance card and individual’s name

Documents to attach If your spouse is not a dependent covered by Sony Health Insurance, a certificate stating your spouse's income is required in addition to the above documents.
However, if your spouse is an insured covered by Sony Health Insurance, no certificate stating your spouse's income is required. Please enter the code, number, and name from your spouse's health insurance card in the field for remarks on the Dependent (Change) Notice. (For detailed information, refer to the example of completed form.)
