The true value of your health insurance card will become apparent in the event of illness or injury. Your health insurance card contains a large amount of fundamental information concerning your health insurance. Learning about your health insurance card can help you learn about the provisions of your health insurance policy.


Date of issuance (date on which coverage begins)

The holder of the health insurance card, who pays the policy premiums, is known as the insured. The insured becomes eligible for coverage on the date he or she joins a workplace (or company) that is part of the Sony Health Insurance Society. Although in certain cases all health insurance cards will be exchanged for new ones, once the insured is issued his or her health insurance card, the date of issuance will remain unchanged from that point forward.
Note: The start date for your insurance coverage may change when you transfer to another workplace or for other reasons.



Date by which the card must be returned (date on which coverage ends)

Your insurance coverage will terminate and you will be required to return the health insurance card to Sony Health Insurance on "the day following the day you leave the company."

You may not use your health insurance card once you are no longer eligible for coverage. Return the card to Sony Health Insurance immediately via your own workplace, then promptly complete the procedures for joining another health insurance plan. This is necessary because neither you nor your family (dependent) will be able to use the insurance provided by Sony Health Insurance from that date forward.
In the event of the death of the insured, his or her health insurance card may no longer be used from "the day following the date of death." His or her family members are requested to undertake the procedures described above.

Refer to "Procedures When Leaving the Company" concerning the medical insurance system for those who have left the company. In addition, Sony Health Insurance will issue "a Certificate of Non-Coverage" if necessary to make it possible to join another health insurance system, so please ask the individual in charge of health insurance matters at your workplace.


* The health insurance card is your healthcare passport
Your health insurance card is a healthcare passport that certifies your coverage under the Sony Health Insurance Society.

When you display your health insurance card at a hospital as proof that you are a member of the Sony Health Insurance Society, you will be charged only a portion of your medical costs.The remainder will be charged to Sony Health Insurance at a later date. In this way, you and Sony Health Insurance together cover medical costs incurred at the hospital. However, if you do not have your health insurance card, the hospital will demand full payment.

In this way, your health insurance card functions not only as identification but as a medical credit card. Be careful to avoid losing your card, and do not loan it to anyone.


If your health insurance card is damaged...

Your health insurance card is acan be reissued if lost or damaged.

If your card has become bent or warped, attach it to "an Application for Reissue of Health Insurance Card" and submit it for replacement. You will be issued a new health insurance card to replace the one rendered unusable.



If your health insurance card is lost... If your health insurance card is lost...

If your card has been "lost," you can apply to have the card reissued. However, you must first submit a notice of lost property at the nearest police station. Unlike a credit card, the validity of your health insurance card cannot be revoked. If someone else uses your health insurance card fraudulently, you may be held responsible.
Also submit a "Notice of Health Insurance Card Loss" if you notice that your health insurance card has been lost upon your retirement from the company.

Required procedures for certain cases

When registering a newborn infant as a dependent Promptly Attach all required additional documents to "the Dependent Notice (Notice of Change)" and submit

Registering family members as dependents (or removing family members from the status of dependents) Within 5 actual working days Attach all required additional documents to "the Dependent Notice (Notice of Change)" and submit

When the name of the health insurance cardholder changes Immediately Submit your "health insurance card" together with "the Notice of Name Change"

When a dependent (family member) is no longer eligible for coverage, as when he or she starts a new job Immediately Submit the family member's "health insurance card" together with "the Dependent Notice (Notice of Change)"

When the insured is no longer eligible for coverage, as when he or she leaves the company or dies Immediately Be sure to return "the health insurance card"
* The health insurance card can no longer be used from the day following the date on which the insured leaves the company.
