Benefits available after losing insurance eligibility
Although your health insurance benefits will cease if you lose your eligibility for coverage on leaving the company, a sudden cessation of benefits for individuals undergoing treatment, hospitalization, etc. before leaving the company may result in catastrophic cost burdens. This system was established to protect the insured in such cases.


Only statutory benefits may be received.

Although all statutory benefits may be received, individuals will not receive any of the additional benefits provided by Sony Health Insurance.



Types and conditions of benefits that will continue to be available

A condition for receiving any benefits is that the insured individual was covered by the plan continuously (i.e.,employed with the company) for at least one year. In addition, you cannot receive benefits from both your new insurance plan and Sony Health Insurance. Select one plan to which to apply for benefits. Application documents are available from the individual in charge of health insurance matters at your workplace.


Benefits that may continue Conditions for receiving benefits Procedures
Injury and sickness Injury and sickness allowance This applies to the following 3 cases:
1. Individuals receiving injury and sickness allowance while with the company
2. Individuals who have completed their waiting period while with the company and for whom the first date of benefits eligibility coincides with their retirement from the company
3. Individuals who have not received an injury and sickness allowance because they received payment of their salary while with the company
To obtain these benefits, attach a physician's certificate to the application for injury and sickness allowance and submit to Sony Health Insurance.

The prescription period during which insurance benefits at left can be received is two years.
Childbirth Childbirth and childcare lump-sum grant When the insured has given birth within six months following the date on which he or she lost coverage eligibility
After giving birth, obtain these benefits by attaching a physician's birth certificate to the application for childbirth and childcare lump-sum grant and submitting to Sony Health Insurance.

<Interim measures>
These benefits are provided when an expectant mother gives birth by May 11, 2007 (or by July 6, 2007 for multiple births).
After giving birth, attach a physician's birth certificate to the application for childbirth allowance and submit to Sony Health Insurance.

Death Funeral expenses (costs) This applies to the following two cases:
1. The insured dies within 3 months following the date on which he or she lost coverage eligibility
2. The insured died while receiving injury and sickness allowance, or after receiving injury and sickness allowance

Attach one of the following to the request for funeral expenses (costs): the burial permit (attach receipt and details), cremation permit; or death certificate (photocopies are acceptable), and submit it to Sony Health Insurance to receive the benefits.
